  • S.E.C. - Articles of Inc. & By-laws  (Partnership/Corporation)
  • Community Tax Certificate "Cedula"
  • Contract of Lease (for Lessor & Lessee)
  • Photos of Business Establishment
  • Locational Clearance
  • Homeowner's Association Clearance
  • Authorization from the real owner of business (in case of representative)
  • PEZA Registration Certificate (for PEZA Member)
  • BOI Registration Certificate (for BOI Member)
  • Sanitary Permit
  • Fire Safety Clearance
  • Engineering Clearance
  • SSS Clearance
  • Other requirements for specific nature of business (case to case basis)
